I can honestly say this career has changed my life. I have been a practicing provider for many years but the huge decision to open my own office in 2019 was a drastic change. I am extremely grateful to have such a supportive husband and family who have cheered me on throughout all of the challenges we have overcome thus far. Opening up the business in December of 2019 was difficult as the coronavirus was beginning to hit our area. I can remember days where we saw only two patients and I feared my business would not survive. After four years, many long nights, thousands of hours, meetings, and much more, I am astounded to see how far Hilltown Healthcare has grown. I started my nursing career after having my second child in 1997. I worked at St Mary's hospital as an RN for 10 years then obtained my Nurse Practitioner Master's in Nursing and started my career as a Family Nurse Practitioner in 2007. I joined Community Care in Berne in 2015 until they abruptly closed the office. I couldn't leave thousands of patients without a primary care provider. I am glad to say we have now officially built our own office from scratch. Not expecting this to occur so soon into opening my business, but I am happy to now begin another step in this journey. I am glad to have such a wonderful staff and appreciate them so much. Fun facts about me; I enjoy spending time with my family, including my four grandchildren, gardening, Buffalo Bills games, and taking rides in the country.

What a dream come true! Working at Hilltown Healthcare has been a huge blessing in my life. Getting to work with my mother, who not only is an amazing NP but also my best friend, is an honor. I could not be more proud of my mother (Jill Martin) for all she has accomplished so far. Her family is beyond proud of her for all she has done for this community. I am happy to say I strongly encouraged my mother to open up her own practice in Berne. There was no doubt in my mind she could achieve anything she put her mind to. As for myself, I absolutely love my job. I always knew I wanted to be a leading part of a business and having it be family owned just makes it that much better. I hope to someday take over the business and follow in my mother's footsteps by helping the healthcare of our community. In 2023 I took on a new role as Health Navigator which I am extremely blessed with, as it allows me to focus on Medicare patients and help them receive the care they deserve both in and out of the office. Fun facts about myself; I enjoy spending my free time riding my motorcycles, hiking, riding atvs, skiing, doing yard work, gardening, going to church, traveling, going to concerts, watching the Buffalo Bills, cuddling my dogs, and most importantly spending time with family and friends. I am so beyond grateful for all this life has given me so far and can't wait to see what the future holds.

Kaden grew up in Boonville, NY and moved to the Capital Region in 2018. Growing up she was interested in science and helping others. She knew she wanted to become a nurse after seeing the exceptional care her grandmother received when she was diagnosed with and battled pancreatic cancer. Kaden completed her Associates of Applied Science in Nursing at Mohawk Valley Community College and both her Bachelors of Science and Masters of Science degrees, at SUNY Polytechnic Institute. Since becoming a Registered Nurse in 2012, Kaden has worked in various settings including inpatient mental health, home care, and case management. She is excited to bring her experience and passion for preventive and patient-centered care to her new role as a Nurse Practitioner in family medicine. One of her favorite aspects of nursing is building meaningful relationships with her patients. She is looking forward to building those relationships in Berne as it fondly reminds me of where she was raised. Outside of nursing she enjoys spending time with her family, parenting, baking, and exercising.

I grew up in Colonie NY. I've lived in East Berne for 12 years. I graduated from the LPN Capital District Career and Technical School. I've been an LPN for 18 years, with experience in Gynecology, IVF Infertility, Endocrinology,Colon & Rectal Surgery and ENT. I now have been given the opportunity to now work at Hill Town Healthcare and help the residents of the hilltown. Outside of work I love spending time with family, friends and my fur babies!! I have one thing on my bucket list and that is to visit Antelope Canyon one day. My goal is to be the best nurse I can and give the best care to our patients. .
LPN
I have been an LPN since 2004. I started my career as a nurse at Teresian House, a local nursing home and continued my career as an allergy nurse for Albany ENT and Allergy for the last 13 years. I am a dedicated nurse who loves learning new skills and helping others. My husband and I have lived in the town of Knox since 2005. We have 3 children, 2 boys and 1 girl. All 3 attend BKW school and we couldn't be more proud of their accomplishments! We love being a part of the Hilltown community, taking part in multiple town wide and school activities as well as active members of our church. I am a summer time kind of girl who loves a variety of outdoor activities including coaching my kids in baseball/softball, playing a variety of sports with them in the yard, gardening, camping in our VW van, boating and lounging on a beach with a good book!
